Non-NHS Services
It is not necessary to make an appointment with the doctor if you require a doctor to sign a form, complete a claim form or provide a certificate. Simply hand the form to a receptionist together with your payment who will pass your form to a doctor for completion. You are requested to phone the surgery after 1-2 weeks to check if your form is ready for collection. Please note that some forms incur a charge.
